This module is an introduction to the management of long term conditions and their monitoring in general practice. On completion of this course you will have developed an understanding of the general practice nurse role in these areas.
This course will cover the management of long term conditions and their monitoring in general practice. In particular you will develop your knowledge and skills in a wide range of conditions including: asthma, cardiovascular disease, chronic obstructive airways disease, diabetes, hypertension, dementia and frailty and 'fatty liver disease'. Through a critical understanding of medicine management and working with patients you will develop an understanding of the general practice nurse role in these areas.
You will benefit from a combination of University teaching and learning in practice with Mentor guidance. Through critical learning you will consider issues relating to leadership, team working, quality and governance, and accountability in order to develop you in your role of the general practice nurse.
